2026 Art Fair on the Square

Step 1

Submit the Required Registration Form at: https://www.mmoca.org/2026form/ 

Step 2

Submit the Required Tax Information via email to artfair@mmoca.org. Every artist that will have a booth at the fair needs to submit Wisconsin Temporary Event Operator/Seller information to MMoCA each year.

IF YOU ALREADY HAVE A WI TAC ACCOUNT NUMBER

(15 digits long and begins with “456”)

Fill out the S-240 Form at https://www.revenue.wi.gov/DORForms/s-240vendf.pdf and send it to artfair@mmoca.org with the subject line AFoTS 2026 Tax Form.

You only need to fill out one section of Part C. 

If you don’t have a Wisconsin Tax Account Number

Visit www.revenue.wi.gov/html/tempsell.html or call the Wisconsin Department of Revenue at (608) 266-2776 to get one so that you can fill out the online form.

You must submit your tax information before the event date or you will not be able to exhibit at the event.
